Rehabilitation

Mountain Valley of Cascadia focuses on patient-centered rehabilitation. With our in-house rehabilitation gym, we utilize a comprehensive approach to physical, occupational, and speech therapies. By doing so, our team can work together with the patient to achieve the highest results possible. In addition to exceptional rehabilitation, we are partnered with an outstanding team of physicians that specialize in rehabilitation and wound care to ensure the most desired outcomes for each patient are achieved. Mountain Valley is pleased to offer rehabilitation therapy seven days a week by highly qualified and licensed personnel.

Long-Term Care

If you do require a more long-term stay option, our skilled nursing facility offers a variety of conveniences and amenities to make Mountain Valley of Cascadia as homelike and comfortable as it can be both for the residents and their families. We offer services in our facility that include individualized attention and care from our experienced and skilled nursing staff such as assistance with activities of daily living from simple guidance with dressing to complete care for all your loved ones’ needs. Mountain Valley also provides all our residents with a personalized activities program run by our activities director. Providing for quality of life and having FUN is in our culture. Our social director is always available to assist our residents and families as they navigate through their journey of long-term care living, answering any questions and providing support along the way. Our goal is for our residents to recover from medical conditions with dignity while receiving the quality, compassionate care they deserve.

Memory Care

Memory care is a long-term care arrangement offered at Mountain Valley of Cascadia specially designed for residents with Alzheimer’s disease, dementia, and other cognitive impairments. Also known as Alzheimer’s care or dementia care, specialized memory care is provided within our facility. Staffed by professionals who are trained to provide this specific care, our program is purposely designed to provide appropriate stimulation and support in a homelike, secure area. This type of dedicated care may be the best option for an older adult with severe memory loss or cognitive decline who is no longer able to live independently.

We pride ourselves on being an industry leader in the provision of care, service excellence, and innovation and have achieved numerous awards along our quality journey including the American Healthcare Associations Bronze, Silver, and Gold Awards and most recently the Nation’s only Presidential Award- the Malcolm Baldridge Award in 2016!  In addition, we are currently only one of 113 skilled nursing facilities in the nation to have continually sustained a “Five Star” Ranking on the Center for Medicare and Medicaid website.
